Verification

SmartMed verification provides insight to the actual medication use. During hospitalisation, knowledge about the medication of the patient is essential for successful treatment. SmartMed has developed a special user-friendly tool to do this efficiently.

Profile

SmartMed Profile supplies all specific patient information with regard to allergies, intolerances, and contraindications for the treatment analysis. For example, an allergy for penicillin or COPD as a contraindication can be registered and managed here, or imported from an EPF. This enables the prescriber to assess warnings about allergies, intolerances and contraindications during the prescribing process, and enables the pharmacist to carry out checks completely and correctly.

Prescribing

SmartMed Prescribing is developed to enable the practitioner to prescribe medication intuitively, safely and efficiently based on formula and assortment. Beside the regular dosage schedules, it is possible to use single, repetitive or combined schedules, both for the actual situation, during hospitalisation or dismissal of the patient. Medical agreements are visually represented, and if required, supported by relevant safety warnings. Medication favourites improve user-friendliness.

Monitoring

SmartMed Monitoring offers all tools required to guarantee patient medication safety. Besides these tools, the option is offered to create your own additional rules and to exert complete control on all medication orders. Customisable filters make it possible to determine relevance of signals and checks based on policy. It enables the practitioner and pharmacist to jointly set the optimal therapy for the individual patient.

Distribution

SmartMed Logistics offers an integral solution for the hospital pharmacist to set up the entire process as purchasing, article and stock management and distribution.. Part of these processes are supplying departments, delivery of patient-specific medication, and external deliveries to clients. The requirements of the Opium Law or the Falsified Medicine Directive are met. Contract management with the suppliers is part of the ordering process. The pharmacist can use special dashboards to monitor and analyse the logistic processes.

Administration

SmartMed administration is a modern administration module, based on “the five rights of medication”: the right patient, the right medication, the right dosage, the right way of administration and the right time. Safety comes first, together with user-friendliness for the nursing staff. During the administration process, the nurse can register one-time administrations, cosigning, patient-specific ordering, and view the medication order, all to realise a flawless administration process.

Use

SmartMed Use fits the SmartMed philosophy that the patient can have an important role in the use of medication. During hospitalisation the choice can be made to give the patient control over the medication use. SmartMed also offers the option to keep on using the medication used at home during hospitalisation. A special app enables the hospital to digitally give the medication file to the patient at dismissal, to improve medication compliance.

Verification

SmartMed enables care institutions to simply obtain information about medication and make it available during hospitalisation and dismissal. Think about acute hospitalisation in a mental health facility, admission in nursing homes, or in institutions for mentally disabled persons. SmartMed makes sure the Actual Medication Overview is available to all employees in the care chain. This prevents spending a lot of time on organising the medication file, and the risk of medication mistakes involved.

Profile

The SmartMed application manages all patient data about allergies, intolerances and contraindications, limiting the risk of unwanted complications when using medication. In the Profile environment, the practitioner has all information about allergies, intolerances and contraindications available. SmartMed generates warnings when medication is prescribed that cause an allergy or intolerance.

Prescribing

SmartMed makes it simple for people in care institutions to keep the medication file up-to-date. In care institutions, working with an up-to-date medication file is often a big challenge. Acute hospitalisations, supporting treatments by general practitioners, and temporary stays require a well maintained Actual Medication Overview. SmartMed also offers the option to have all practitioners work in a single prescribing system, preventing overwriting, and the client can be treated based on a single file.

Monitoring

Monitoring the prescribing of medication has been set up clearly and simply in SmartMed. The prescriber is given warnings, generated by using local medication registers. SmartMed provides a dialogue between prescriber and pharmacist to assess these warnings and to come to the best pharmacotherapeutic treatment. Because SmartMed is working in a single, closed environment, it is possible to professionally organise warnings, substitution propositions and checks.

Distribution

Prescribing, distributing, and administration can be organised in one environment in SmartMed. In SmartMed the pharmacist can manage the logistics of the delivery of medication, selected during prescribing. All logistical aspects, such as defining departments and stock, support with opiate registration, and purchasing are part of the standard activities of SmartMed.

Administration

SmartMed offers the nursing staff the option to completely manage the administration process digitally. The pressure on nursing staff is high. Often, the attention in care institutions is not primarily aimed at supplying and administering medication. SmartMed Administration makes sure that the attendants can follow the process of administration efficiently and transparently. Important aspects are: unequivocal administration overview, option to order from the administration process, adequate information when transferring. SmartMed has conceived the Administration Module based on this scope, aimed at creating an efficient administration process that can reduce the number of incidents to a minimum.

Use

SmartMed offers care institution the option to support the independent patient in the medication process. Self-management of medication by the client is possible. Use and administration registration of medication can be done by the client through an app. This patient app is uniquely developed by SmartMed. The app makes real-time use of the same data created during prescribing. Besides, the app can share health information of the client with the practitioner. For this, the user has access to authorisation settings to select practitioners for access to medical data.

 

Apothecary

Verification

SmartMed offers a unique solution for the public pharmacy to manage the Current Medication Overview (AMO) as a director. The public pharmacy is an essential link in the medication process for the individual patient. SmartMed has processed and arranged the LSP information in an efficient manner, so that it quickly becomes clear to the therapist which medication is up-to-date. The pharmacist can also get real-time access to all medication provided by SmartMed, as given during a hospital stay. By working with verification via SmartMed, the pharmacist distinguishes himself as a healthcare professional in supervising the patient in the area of medication use.

Profile

In SmartMed, the management of ICAs (Intolerances, Contra-indications and Alergies) is arranged according to the NICTIZ standard. This gives the pharmacist the opportunity to store all the information needed to describe ICAs in the application. Of course, active monitoring then takes place. By using different colors, SmartMed quickly provides insight into the severity of the warnings that are related to ICA.

Prescribtion

SmartMed offers the pharmacy the possibility to process recipes, in new terminology "provision requests". Because SmartMed defines an AMO as well as a provision request separately from each other, the pharmacist can take on both the substantive and logistical role.

Monitoring

SmartMed has created a control environment in which the public pharmacist can optimally use his / her role as a healthcare provider. The G-standard warnings are standard part of the application. In addition, the pharmacist can define control rules himself in order to monitor selected medicines, combinations or patient groups. By working in a balanced healthcare chain, with the management of ICAs and own control rules, the pharmacist can fully realize his / her care role.

Distribution

The SmartMed distribution module focuses on efficient logistics and seamless cooperation with wholesalers. Stock and assortment management are essential for a well-running pharmacy. Where the pharmacist is confronted with preference policy and drug shortages every day. This makes it necessary to have a well-managed distribution environment available. SmartMed anticipates that inventory management can take place at cluster level in the future. This has been taken into account in the design of the application.

Administration

SmartMed offers the patient a tool to support the administration of medication in the home situation. The home care employee or informal carer can use the patient app, in which all information about administration times is displayed. Information about drug use provided by the general practitioner can be read in the App via the digital leaflet.

Use

The GP, if authorized by the patient, can get access to the use of medication by the patient in SmartMed. Applications can be registered. Alerts for forgotten donations are built in. In this way, the GP receives information about adherence and can thus better monitor pharmacotherapy. This can be taken into account in the decision making to adjust current medication.

 

General Practitioner

Verification

With the verification module, SmartMed provides quick insight into the current status of the patient's medication. It is of great importance for general practitioners that information about medication is up-to-date. The SmartMed application offers the possibility to consult and send the Current Medication Overview both prior to a consultation and when transferred to a hospital or healthcare facility. This contributes to the reduction of medication incidents.

Profile

SmartMed offers the general practitioner the possibility to manage medication-related allergies & intolerances of the patient. SmartMed actively monitors this. The registration of allergies, intolerances and contraindications is organized according to the most current NICTIZ standards. With this information the GP can guarantee high quality of pharmacotherapy, aimed at patient safety.

Prescribe

With SmartMed, the medication process is organized according to the NICTIZ standard. It is government policy that the medication agreement and the request for provision be split. This promotes proper maintenance of the Current Medication Overview (AMO), independent of the delivery of medication by the pharmacy. This medication appointment can last for a number of years (use to stop) while the delivery of medication is limited to periods that have been agreed in consultation between the health insurer and the pharmacist. This way SmartMed ensures that the medication file is complete, regardless of where the patient is located.

Monitoring

In SmartMed, medication control is organized in such a way that the GP is enabled to assess warnings during prescribing. This concerns all warnings included in the G standard, over- and under-dosing, interactions, etc. It is also possible to consult with the pharmacist for the assessment of warnings via a chat function. As a result, pro-active decision making takes place with regard to pharmacotherapeutic considerations.

Much of this information can be made available by the pharmacist in the digital patient environment in SmartMed. Every patient is given a secure environment in the SmartMed cloud, in which the pharmacist can place specific administration information. In addition, SmartMed offers the possibility to monitor adherence, based both on delivery date / numbers and on delivery information from the patients App.
